Delivering your Completed Forms

You may bring your completed forms with you on the day of your first appointment, which will speed up your appointment, or fax them to us ahead of time. Please phone us for our fax number. We don’t encourage scanning and sending your completed forms via email, due to the lack of security that email offers.
